As sad as it is, there has to come to a point that someone needs to step in and take the license to drive away. My grandmother was legally blind and kept getting her license renewed. She was on all types of medication every day that we had to write the names of on the caps with a Sharpie so she could see them and would take them correctly. My mother stepped in and talked to her doctors and she was removed from the road. It meant more work for us as we took her to the doctor, church, and grocery shopping, but we knew that she was safe and the community was safer. It devastated her for a while, but she did realize within her heart that it was for the best.
